The form GAO-21: Request for an Accounting Correction has been updated and is now available on the GAO website. This updated form is for immediate use by agencies for all future requests. This form has had significant changes which include: 

• Format and layout
• Language and verbiage
• Checkbox options reduced

All of these changes were implemented to help eliminate confusion that frequently occurred with the previous version of the form. When using the new form, read the instructions thoroughly to ensure the form is completed correctly.
