New ADOA AFIS MAINFRAME ACCESS REQUEST form

The ADOA-ASET Security/Privacy/Risk (SPR) team has created a form for new AFIS end users to complete. The AFIS MAINFRAME ACCESS REQUEST (AFIS Only) form replaces the ADOA MAINFRAME ACCESS REQUEST form. The new form has more detailed instructions, fewer fields, and a more efficient and secure verification word process. This change becomes effective on 4/7/2015. Any new AFIS end user requests submitted with the prior form will be processed through 4/8/2015.

The AFIS MAINFRAME ACCESS REQUEST (AFIS Only) form does not replace the AFIS SECURITY AUTHORIZATION FORM (GAO-96), which is still required to be sent to ADOA-GAO Security. 

The following is the workflow for a new AFIS end user request:

  1. Agency submits a completed Form GAO-96 for the new AFIS end user to ADOA-GAO Security either by fax to 602-542-7066 or email to [email protected].
  2. Agency submits a completed AFIS MAINFRAME ACCESS REQUEST (AFIS Only) form to ADOA-ASET SPR either by fax to 602-542-0095 or email to [email protected] .
  3. ADOA-ASET SPR sends an email with a randomly assigned secret verification word to the new AFIS end user’s state issued email address listed on the AFIS MAINFRAME ACCESS REQUEST (AFIS Only) form. The verification word will be required to confirm the end user’s identity to reset their AFIS password.
  4. If the AFIS end user requests to change their verification word, they will need to either send an email to [email protected] with their name, agency, and new verification word from their state issued email address or call ADOA ASET Security at 602-542-0257. If the end user calls, they will be required to provide their current assigned verification word or the last four digits of their SSN.
  5. ADOA-GAO Security sends a final notification email to the agency’s AFIS Security Administrator indicating the new AFIS end user’s request has been completed. 
  6. The AFIS Security Administrator notifies the new AFIS end user that their access has been established.
  7. The new AFIS end user calls the ADOA Help Desk at 602-364-4444, option #2 to have their password reset. The end user will need to be in the AFIS login screen and provide their secret verification word.
