New claim form, GAO-504NA, has been published on the GAO website.  This form will be used for handwrite requests required by law to be paid between July 1st through July 7th.  Due to the cutover to the new AFIS system, there will not be a financial system available to enter claims during this estimated period of time.  

Agencies must submit the form GAO-504NA along with a written letter of request to GAO no later than June 15, 2015.  Handwritten warrants that use appropriated funds will be issued only if the appropriation and departmental budget are loaded in new AFIS.  Any exceptions will require approval from the State Comptroller.
