Beginning 11/4/19, the GAO will begin rejecting VCCD1, VCMD1 documents and GAO ACH Setup Forms that do not have required fields completed and/or entered. A couple of the required changes are noted below.

• GAO W-9 and GAO-618 Forms must be a version dated 10/2018 or later
• Contact information must be entered (email address and/or phone number are now required)

We have also separated the GAO W-9 – State of Arizona Substitute W-9 Form and the GAO-618 ACH Authorization Form. If you are providing either form to a vendor, please be sure you are providing a form version dated 10/2018 or later or the form will be returned and/or rejected.

The most recent version of both forms can always be found on the GAO website under Publications and then Forms sections.
