New Web Form for APP Ticket Creation
In an effort to improve the APP Ticketing Process, SPO has launched a new Web Form that can be used to enter issues. Instead of sending an email to the APP email, please fill out the Web Form and it will automatically create a new ticket in the Cherwell Ticketing System. The link to the Web Form will also be posted on the APP website under both the Agency Support Tab and also under the Supplier Support Tab. Brief instructions on filling out the Web Form are below.
Users from the following Agencies will continue to follow their Agency’s procedures for getting help and support:
AHCCCS
Attorney General’s Office
Child Safety
Corrections
Economic Security
Emergency and Military Affairs
Environmental Quality
Game & Fish
Health Services
Industrial Commission
Transportation
Veterans’ Services
If you have an agency subject matter expert that has been forwarding issues to the Help Desk please continue to utilize their expertise.
Web Form Instructions
The following information needs to be entered in the Web Form:
- Your Name
- Phone Number
- Title of Problem
- Description of Issue/Request
- Classification (Select from the drop-down menu)
- Select “State Agency”
- Set a Priority [1 = High (prevents you from conducting State business)… 4 = Low (can wait a few days for an answer)]
If you need to include an attachment on the Web Form, follow these instructions:
Attachment Instructions
- At the bottom left side of the Web Form, select the box “Choose File”
- Select your file to be uploaded
- At the bottom right side of the Web Form, select “Upload”
- If you are done filling out the Web Form then select Submit
After you hit the Submit Button, you will see a message and you will receive an email that acknowledges that your request was submitted. You will also receive an email that will provide you with your assigned ticket number.
