Security operations involving the Central Services Bureau (CSB) and its client agencies will change and a new form, the GAO-3CSB is being introduced to support these changes.

The GAO-3CSB is a two-part form.  The top half of the form is designed for and used by CSB’s client agencies and includes the same certifications concerning disbursements and transfers required by the Form GAO-3DT from all those approving such transactions.  This section of the form, once completed, is to be delivered to and retained by the CSB.

The bottom half of the form is to be completed by CSB personnel and contains modified certifications reflecting their somewhat unique role in processing, though not approving, disbursements and transfers.  This section of the form, once completed, is to be delivered to and retained by GAO Security.
