Updated Platinum Pass Application (GAO-624)

  • The new version is located here: /sites/default/files/gao-624.pdf.  Please discard all prior versions. 
  • Includes a reminder to the employee to ensure their address is accurate by verifying in Y.E.S. (www.yes.az.gov) prior to submitting an application since transit cards will be mailed.
  • The Platinum Pass Application has been updated with an Agency section to complete once they have assigned the transit card in HRIS.  Reminder: these applications serve as the Payroll Deduction Authorization and must be signed by the employee prior to the agency assigning a transit card.  Agencies do not need submit the applications to the GAO. 
  • Option for the employee to “Opt Out” of the transit card program

Agency Platinum Pass Issuance Procedures

  • The assignment, replacement, and cancelation processes have been posted to the GAO website within the Agency Payroll Guide located here: /sites/default/files/APG_IV_D_Transit_Cards.pdf   
  • The technical HRIS Training for the Agency Transit Card Processor (ATCP) job role will be posted to the HRIS website shortly.

New Distribution Procedures

  • The mass renewal will be the last platinum pass distribution through the Agencies. 
  • Beginning October 1, 2016, any State employee that was not included in the mass renewal (a new hire, or an employee with no platinum pass activity since January 2015) can apply for a new pass by submitting a Platinum Pass application (Form GAO-624) to their agency directly.  HRIS power users assigned the Agency Transit Card Processor Role will then be able to assign the employee a new platinum pass directly via HRIS.  The agency would not need to forward the application to the GAO.  Agencies would retain the form in their files.  GAO Central Payroll would then review the new transactions entered and, on the next business day, mail the pass directly to the employee’s address of record in HRIS.  Distributing passes in this fashion will help reduce employee wait time statewide from 7 business days to 4 business days while also reducing agency distribution costs.
