BFY25 (Received in FY26) Administrative Adjustment & Extension Requests

The General Accounting Office (GAO) has introduced a new process for administrative adjustment requests and extension requests. To submit an administrative adjustment request and/or an extension request, use the new Administrative Adjustment & Extension Requests Google Form. The Administrative Adjustment Request PDF form will no longer be available nor accepted for submissions as of close of business today, April 30th, 2025.

 

Starting May 1st, 2025, to submit an administrative adjustment request or an extension request:

  1. Visit the GAO website and navigate to the Forms page under Publications: https://gao.az.gov/publications/forms
  2. Locate form GAO-AAR, Administrative Adjustment Request and click on the link.
  3. Follow the instructions provided on the Google Form.
